Executive teams do not fail for want of intelligence or experience. Under sustained pressure, uncertainty and complexity, they gradually lose access to the judgement they are entirely capable of exercising — and at executive level, the consequences are organisational rather than personal.

What actually goes wrong

Rarely a failure of capability. Almost always a failure of the conditions under which capability is exercised.

By the time an executive team is under real pressure, the deterioration has usually already begun. Deliberation narrows. Dissent quietens. The same three voices carry the room. Decisions get taken early to relieve the discomfort of not having decided, and are then defended long after the evidence has moved.
